COVID-19 EVICTION MORATORIUM - 2020
By filling out this form, many tenants across New York State can ensure they are protected from eviction until August 31st.
The COVID-19 Emergency Eviction and Foreclosure Prevention Act of 2020 has extended the eviction moratorium for all tenants until August 31st for those who fill out this Tenant Declaration of Hardship form.
This form MUST be completed and filed in order to insure your protection under the COVID-19 Emergency Eviction and Foreclosure Prevention Act. Please click the button below and submit your information. It will be directly sent to the necessary parties.
NOTICE: You are signing and submitting this Declaration under penalty of law. That means it is against the law to make a statement on this Declaration that you know is false.
Anyone who is behind on rent and falls into one of the two protected categories (either they have suffered financial hardship or face health risks because of COVID-19) should fill out the form as soon as possible, even if you don’t currently have a case in housing court.
By filling out this form, provided by the Housing Justice for All Coalition, you can automatically submit your Declaration of Hardship to your landlord's email, the local courts, and to your own email for your records.
You can also print, fill out and mail the Declaration of Hardship to your landlord yourself, using the printable forms linked below:

Make sure you send the Declaration to your landlord using certified mail. Save the tracking number and a copy of the filled out and signed Declaration for your records.
If you know anyone who is struggling to pay rent, be sure to share this with them!
